Health Services

   There are several health forms required by the nurse as established by the State of Illinois. They are as follows:

Certificate of Child Health Examination

 

1.     Incoming freshmen students must present a current physical examination dated within 12 months of the first day of school.  Students who do not meet these requirements will be excluded from school as of October 15, 2010.

2.     Please remember to complete the “Health History” and sign the form prior to your appointment with your health care provider.

3.     If your child is interested in participating in athletics, an additional copy of the Certificate of Child Health Examination or the IHSA IESA Pre-participation Examination must be submitted to the Athletic Department.

4.     All children entering school in Illinois shall present evidence of immunity against the following:

 

REQUIRED IMMUNIZATIONS

ILLINOIS RULES AND REGULATIONS

Diptheria, Pertussis & Tetanus

3 or more doses with last dose on or after 4th birthday.  If 10 years has elapsed since last dose an additional dose of TD is required

Polio

3 or more doses with last dose on or after 4th birthday

Measles, Mumps & Rubella

2 doses (1st dose must be after 1st birthday)

Hepatitis B Series

3 doses

Varicella (beginning 2011)

1 dose on or after 1st birthday

 

EXCEPTIONS:  Medical and religious exemptions are acceptable with a written doctor’s note or a written statement with a full explanation of the parent’s religious beliefs.

 

Medication Administration

 

A student may carry an Inhaler, Epi-pen or Insulin as needed providing a Permission Form to Administer Prescription Medication is completed by your physician, signed by parent/guardian and on file in the Nurse’s Office.

 

1.     Prescription Medications may be administered during school hours with a completed Permission Form to Administer Prescription Medication on file in the Nurse’s Office.

·         The Permission Form to Administer Prescription Medication MUST be signed by both parent/guardian and licensed provider.

·         All prescription medications MUST be in the original pharmacy container with a current pharmacy label

2.     Over the Counter Medications may be administered during school hours with a completed Permission Form to Administer Over The Counter Medication on file in the Nurse’s Office.

·         A limited supply of Tylenol, Advil, Midol, Tylenol Cold, Tums, Pepto Bismol and Benadryl are available to students who have their permission form completed on file in the Nurse’s Office.

·         We encourage you to send in any over the counter medications that your child frequently uses as the supply in the Nurse’s Office is limited.

·         All over the counter medications MUST be in the original store-labeled container, “checked” or listed on the permission form and labeled with your child’s first and last name.  These medications will be kept in a locked cabinet and used only for your child.
